응급실 간호사의 언어폭력경험이 전문직 삶의 질에 미치는 영향: 직무스트레스의 매개효과

Abstract

The purpose of this study was to investigate the relationships between verbal abuse experience, the professional quality of life, and job stress among emergency nurses and to identify the mediating effect of job stress on the professional quality of life. The subjects of this study comprised 121 emergency nurses working for university and general hospitals located in ‘D’ and ‘K’ regions, and the data were collected from December 1st, 2017 to February 1st, 2018. The collected data were analyzed through frequency analysis, t-test, ANOVA, Scheffe, Pearson correlation coefficient, and three-step mediated regression analysis using SPSS 23.0 software. The obtained mean scores from the self-reporting questionnaire were 2.97 on the verbal abuse experience, 3.02 on the professional quality of life, and 3.90 on job stress. The experience of verbal abuse according to the general characteristics showed statistically significant differences with regard to total working period (t=3.69, p=.028), working period in ER (t=7.11, p=.001), total number of patients per nurse (t=-4.20, p=.001), and hospital type (t=-4.08, p=.001). Moreover, the strong influencing factors of the professional quality of life were the total number of patients per nurse (t=2.47, p=.015) and hospital type (t=2.85, p=.012). Job stress showed significant differences according to the number of months working in the current position (t=9.76, p=.000) and total number of patients per nurse (t=-2.26, p=.026). The results of correlations analysis between verbal abuse experience, job stress, and the professional quality of life presented that verbal abuse experience has negative associations with the professional quality of life (r=-0.53, p<.001); similarly, job stress is also negatively related with the professional quality of life (r=-0.47, p<.001), whereas verbal abuse experience has a positive relationship with job stress (r=0.66, p<.001). Job stress revealed significant effects on verbal abuse experience and the professional quality of life, with their explanatory powers being 43% and 29%, respectively, indicating partial mediator effects in the relationship between three variables. In conclusion, it is confirmed that verbal abuse experience and job stress in emergency nurses reduce the professional quality of life, and their relationship manifested partial mediating effects. It is suggested, therefore, that it is necessary to decrease verbal abuse experience and job stress to further improve the professional quality of life in emergency nurses.

본 연구는 응급실 간호사의 언어폭력경험, 전문직 삶의 질 및 직무스트레스의 상관관계를 파악하고, 언어폭력경험이 전문직 삶의 질에 미치는 효과에서 직무스트레스 매개효과를 규명하기 위한 서술적 조사연구이다. 연구 대상은 D 지역과 K 지역 소재의 상급종합병원, 종합병원에서 근무하고 있는 응급실 간호사 121명을 대상으로 하였고, 자료수집 기간은 2017년 12월 1일부터 2018년 2월 1일까지였다. 통계처리는 SPSS 23.0을 이용하여 빈도 분석과 t-test, ANOVA, Scheffe's test, Pearson's correlation coefficients, three-step mediated regression analysis를 사용하였다. 대상자의 언어폭력경험은 평균 5점 만점에 2.97점, 전문직 삶의 질 평균 5점 만점에 3.02점, 직무스트레스 평균 5점 만점에 3.90으로 나타났다. 대상자의 일반적 특성에 따른 언어폭력경험의 차이는 임상경력(t=3.69, p=.028)과 부서경력(t=7.11, p=.001), 간호사 1인당 환자수(t=-4.20, p=.000), 근무병원 유형(t=-4.08, p=.001)에서 통계적으로 유의한 차이가 나타났다. 전문직 삶의 질에 영향을 미치는 요인을 분석한 결과 간호사 1인당 환자수(t=2.47, p=.015)와 근무병원 유형(t=2.85, p=.012)에서 통계적으로 유의한 차이가 나타났다. 직무스트레스에 영향을 미치는 요인을 분석한 결과 부서경력(t=9.76, p=.000)과 간호사 1인당 환자수(t=-2.26, p=.026)에서 통계적으로 유의한 차이가 나타났다. 응급실 간호사의 언어폭력경험이 전문직 삶의 질에 유의한 음의 상관관계가 있고(r=-0.53, p<.001), 언어폭력경험과 직무스트레스에 유의한 양의 상관관계가 있었다(r=0.66, p<.001). 그리고 전문직 삶의 질과 직무스트레스도 유의한 음의 상관관계로 나타났다(r=-0.47, p<.001). 응급실 간호사의 직무스트레스가 언어폭력경험과 전문직 삶의 질에 유의한 영향을 주었고 각각 설명력은 43%, 29%였으며, 이들 간의 관계에서 부분매개효과를 보였다. 결론적으로 언어폭력경험과 직무스트레스가 전문직 삶의 질에 영향을 미치고 있으며, 이들 관계에서 직무스트레스가 유의한 매개효과가 있는 것으로 확인하였다. 따라서 응급실 간호사의 전문직 삶의 질을 높이기 위한 언어폭력경험과 직무스트레스를 줄이는 것이 필요함을 알 수 있다.
