일시적 회장루 보유자를 위한 건강관리 역량강화 프로그램 개발 및 효과 검증

Abstract

Purpose: The purpose of this study was to develop and evaluate a health care empowerment program and verify its effectiveness for temporary ileostomy patients.

Method: A randomized control group pretest-posttest design was used for this study; the data was collected from June 2019 to September 2020, and the subjects were patients who have received a surgery for the temporary ileostomy at A university hospital in Daegu city, Korea. The people who participated in the study were randomly assigned to two groups: the experimental group(n=15), which participated in the health care empowerment program, and the control group(n=15), which was provided with a general program. Data were analyzed through descriptive statistics, chi-square, fisher exact test, linear by linear association test, Mann-Whitney U test, Wilcoxon test using the SPSS 20.0 program.

Results: The major results of this study were that the experimental group showed significantly higher post-test scores in health care empowerment than those of the control group(p=.003). Also, the experimental group showed significantly higher post-test scores in stoma self-management knowledge than those of the control group(p=.008). Furthermore, the experimental group showed significantly higher post-test scores in stoma self-management behavior than those of the control group(p=.002). Lastly, the experimental group showed significantly higher post-test scores in degree of peristomal skin damage than those of the control group(p<.001).

Conclusion: The research presented that the health care empowerment program is effective in improving health care empowerment, self-management knowledge, self management behavior, as well as improving degree of peristomal skin damage. Therefore, in the present situation where there is a lack of systematic health care empowerment program for temporary ileostomy patients, the results of this study can be used to develop and standardize a systematic health care program. For further research, strategic development that can continuously facilitate health care empowerment of patients with temporary ileostomy is highly recommended.

목적: 본 연구는 일시적 회장루 보유자를 위한 건강관리 역량강화 프로그램을 개발하고 적용하여 그 효과를 검증하기 위한 목적으로 실시되었다. 연구는 무작위 대조군 전후 실험설계를 실시하였으며, 자료수집 기간은 2019년 6월부터 2020년 9월까지 이루어졌으며, 연구대상은 D 광역시 K대학병원에서 일시적 회장루 수술을 받은 환자를 대상으로 하였으며 실험군 15명, 대조군 15명이었다.

방법: 자료분석은 SPSS Version 22.0 program을 이용하여, 사전 동질성 검증은 chi-square test, Fisher's exact test, linear by linear association test, Mann-Whitney U test 가설검증은 chi-square test, Fisher's exact test, Wilcoxon sign rank test, linear by linear association test 신뢰도 검사는 Cronbach's α검증을 실시하였다.

결과: 역량강화는 건강관리 역량강화 프로그램에 참여한 실험군은 대조군보다 역량강화 점수가 유의하게 높은 것으로 나타났고(P=.003), 장루 자가관리 지식 점수가 유의하게 높은 것으로 나타났고(P=.008), 장루자가관리 수행 점수가 유의하게 높은 것으로 나타났다(P=.002). 또한 건강관리 역량 강화 프로그램에 참여한 실험군은 대조군보다 장루주위 피부손상 정도가 유의하게 낮은 것으로 나타났다(P<.001).

결론: 건강관리 역량강화 프로그램은 일시적 회장루 보유자의 건강관리 역량강화, 장루 자가관리 지식, 장루 자가관리 수행, 장루주위 피부손상 정도를 향상시키는 효과적인 중재로 검증되었다. 따라서 일시적 회장루 보유자를 위한 체계적인 건강관리 프로그램이 거의 없는 현 상황에서 본 건강관리 프로그램을 표준화하여 간호현장에 적응함으로써 일시적 회장루 보유자의 합병증을 감소시키고 삶의 질을 증진시키는 간호중재로 활용할 수 있을 것이다.
