정신건강 위기 척도 개발 :Engel의 모델을 중심으로

Abstract

Purpose: This study developed a assessment scale for evaluating the crisis for mentally ill patients at the community crisis intervention site and tested its validity and reliability. Based on Engel’s biopsychosocial model, a hybrid model was used to analyze the concept of the mental health crisis, identify its attributes, and develop a mental health crisis assessment scale based on the derived attributes.

Method: Research design is a methodological study that develops a mental health crisis assessment scale and tests the validity and reliability of the tool. The subjects were mental health welfare center workers with more than one year of practical experience including nurses, social workers, clinical psychologists, and general administrators in the field of mental health.

Results: The initial 27 items were derived through literature review and focus group interviews. As a result of conducting a content validity test by a group of 10 experts, 23 questions were derived. A preliminary survey was conducted on 20 workers at mental health welfare centers. Data were collected from 282 workers at mental health welfare centers based on 23 questions developed after the preliminary survey. For data analysis, item evaluation and validity test on composition were performed. It went through the process of confirmatory and exploratory factor analyses. In the confirmatory factor analysis, the model’s goodness of fit was performed, and the following results that met the standard of suitability were derived: 2.464, GFI .901, RMSEA .077, CFI .935, and TLI .911. Therefore, as the final analyzed factors for the mental health crisis assessment scale, 4 components and 16 items in the sub-categories were finally chosen.

Discussion: The final questions were 4 questions for physical factors, 3 for social factors, 4 for suicidal behavior factors, and 5 for mental state factors. As for the reliability of the entire questions, Cronbach’s alpha value was .88, securing the reliability.

As a result, a total of 16 questions on mental health crisis assessment measures were developed. The mental health crisis assessment scale attempted a comprehensive mental health crisis scale beyond the existing evaluation tool consisting of a single perspective to evaluate additional factors including the possibility of suicide. It is believed that this can be reflected in the implementation and design of preventive measures. In conclusion, these findings will provide important support to crisis management practitioners who need to make quick and accurate decisions with a multidimensional approach in biopsychosocial aspects for subject’s crisis evaluation.

본 연구는 지역사회 위기개입 현장에서 정신질환자에 대한 위기를 평가하기 위한 척도를 개발하고, 타당성과 신뢰성을 검정하기 위해 수행되었다.

연구를 위해 Engel (1977)의 생물심리사회적 모델을 토대로 혼종모형을 이용하여 정신건강 위기에 대한 개념분석을 하여 속성을 파악하고, 도출된 속성을 기반으로 정신건강 위기 척도를 개발하는 과정을 거쳤다. 연구 설계는 정신건강 위기 척도를 개발하고, 개발한 도구의 타당도와 신뢰도를 검정하는 방법론적 연구이다. 연구대상은 실무 경력이 1년 이상 된 정신건강복지센터 종사자로 정신건강간호사, 정신건강사회복지사, 정신건강 임상심리사, 일반종사자로 하였다.

연구진행 방법은 문헌고찰과 포커스 그룹 면담을 통해 초기 27개 항목을 도출하였다. 이를 10명의 전문가 집단에 의한 내용타당도 검정을 한 결과, 23개의 문항이 도출되었다. 이후 정신건강복지센터 종사자 20명을 대상으로 예비조사를 시행하였다. 예비조사 후 개발된 23개의 문항을 282명의 정신건강복지센터 종사자를 대상으로 자료 수집을 하였다.

자료 분석은 문항평가 및 구성타당도 검정을 시행하였다. 확인적 요인분석과 탐색적 요인분석 과정을 거쳤다. 확인적 요인분석에서 모형적합도 검정을 실시하였고 연구모형의 적합도는 CMIN(χ2)/df 값이 2.464, GFI .901, RMSEA .077, CFI .935, TLI .911로 적합기준치에 부합하였다. 따라서 정신건강 위기 척도에 대한 최종 분석된 요인으로는 4개의 구성요인과 그에 따른 하위영역의 16개의 문항이 최종 결정 되었다. 최종 문항은 신체적 요인에 4개 문항, 사회적 요인에 3개 문항, 자살행동요인에 4개 문항, 정신상태 요인에 5개 문항이다. 전체 문항의 신뢰도 Cronbach's α 값은 .882로 신뢰도를 확보하였다.

이상의 결과로 총 16개의 문항의 정신건강 위기 척도가 개발되었다.

본 연구에서 개발된 정신건강 위기 척도는 기존의 단일관점으로 이루어진 평가도구를 넘어 자살 가능성을 포함한 추가적인 요인들을 평가할 만큼 포괄적인 정신건강 위기 척도에 대한 연구를 시도하였다. 이는 예방대책의 시행 및 설계에도 반영될 수 있을 것으로 사료된다. 또한 대상자의 위기 평가에 있어서 생물심리사회적 측면의 다차원적인 접근방식으로 신속하고 정확한 결정이 필요한 위기관리 실무자에게 중요한 지원을 제공할 것으로 사료된다.
