요양병원 간호사대상 온라인 커뮤니티기반 치매환자 행동심리증상 관리역량 강화 교육프로그램 개발 및 효과검증 :목표달성이론을 중심으로

Abstract

본 연구는 요양병원 간호사를 대상으로 온라인 커뮤니티기반 치매환자 행동심리증상 관리역량 강화 교육프로그램을 목표달성이론을 중심으로 개발하고 그 효과를 검증하기 위해 실시되었다. 연구방법은 비동등성 대조군 전후 설계로, 2022년 11월 28일부터 12월 25일까지 수행되었고, D시 및 K도에 소재한 총 4개의 요양병원 중 A, B 요양병원에서 실험군 23명, C, D 요양병원에서 대조군 24명의 자료를 수집하였다. 실험군은 온라인 커뮤니티를 기반으로 상호작용과 집단교류(동영상, 사례기반 팀학습) 및 개별교류(목표달성 상호확인, 개별 피드백)를 적용한 치매 행동심리증상 관리역량 강화 교육프로그램을 매주 60-80분, 총 4주간 제공하였다. 대조군은 치매환자 행동심리증상 관리역량 강화에 대한 교육책자를 제공하였다. 자료분석은 SPSS/WIN 23.0 프로그램을 통해 연구 대상자의 동질성은 Chi-square test, Fisher′s exact test, t-test로 검증하였고, 종속변수의 동질성 검증은 Independent t-test로 분석하였으며, 가설검증은 Independent t-test로 분석하였다. 연구결과, 실험군과 대조군 간 공감적 이해(t=2.18, p=.035), 의사소통행위(t=3.49, p=.001), 치매환자 행동심리증상 대처역량(t=3.45, p=.001)은 유의한 차이가 있는 것으로 나타났다. 본 연구결과, 목표달성이론을 중심으로 한 온라인 커뮤니티기반 치매환자 행동심리증상 관리역량 강화 교육프로그램은 요양병원 간호사의 치매환자 행동심리증상 관리역량을 향상시킬 수 있는 효과적인 중재임이 확인되었다. 추후 요양병원 간호사의 치매환자 행동심리증상 관리를 위한 직무향상 교육에 활용되기를 기대한다.

This study was conducted to develop an online community-based education program of improving behavioral psychological symptoms of dementia (BPSD) management and to test the effect of program for long-term care hospital nurses based on the goal attainment theory. The study design was non-equivalent control group pre-post-test. Data collection was performed from Nov. 28 to Dec. 25, 2022 and a total of 47 nurses had participated in this study (23 in the experimental group and 24 in the control group). The program was applied to the experimental group for 4 weeks based on online community for 60-80 minutes of group transaction (video, case-based team learning) and individual transaction (goal achievement mutual confirmation, individual feedback). The control group was provided with educational brochures. Data analysis was conducted using SPSS/WIN 23.0 program, using Chi-square test, Fisher's exact test, t-test and Independent test. The findings of this study showed that there were significant differences in empathic understanding (t=2.18, p=.035), communication behavior (t=3.49, p=.001), and managing competence of behavioral and psychological symptoms of dementia (t=3.45, p=.001) between the experimental group and the control group. Based on the results of this study, it was confirmed that Online Community-based Education Program of Improving Behavioral Psychological Symptoms of Dementia Management based on the goal attainment theory is an effective intervention for long-term care hospital nurses. It is expected to be used for job improvement education for the management of dementia behavioral psychological symptoms of long-term care hospital nurses.
