혈액투석 환자의 감사성향과 스티그마가 극복력에 미치는 영향

Abstract

This study aimed to identify the impact of gratitude and stigma on the resilience of patients undergoing hemodialysis. The study participants were 181 patients undergoing hemodialysis. Data were collected using a structured self-report questionnaire from January 15 to March 28, 2025. The collected data were analyzed using a descriptive analysis, t-tests, ANOVA, Scheffé test, Pearson’s correlation coefficients, and hierarchical regression analysis. The participants’ average resilience score was 2.77 out of 5. Among general characteristics, employment status (t = 2.25, p = .027), perceived economic status (F = 6.44, p = .002), and perceived health status (F = 23.63, p < .001) showed significant differences in resilience scores. Resilience was significantly positively correlated with gratitude (r = .724, p < .001), and significantly negatively correlated with stigma (r = –.479, p < .001). As a result of a hierarchical regression analysis, gratitude (β = 0.58, p < .001) and stigma (β = –0.19, p = .001) were found to be significant predictors of resilience, explaining 35.0% of the variance. In conclusion, strategies to enhance resilience in patients undergoing hemodialysis should focus on developing and implementing programs that can promote gratitude and reduce stigma.

본 연구는 혈액투석 환자의 감사성향, 스티그마가 극복력에 미치는 영향을 파악하고자 하였다. 본 연구 대상자는 혈액투석 환자 181명이었다. 자료는 2025년 1월 15일부터 3월 28일까지 구조화된 자가 보고식 설문지를 사용하여 수집되었다. 수집된 자료는 빈도와 백분율, 평균과 표준편차, t-test, ANOVA, Scheffé test, Pearson’s correlation coefficients, Hierarchical regression analysis를 사용하였다. 대상자의 극복력 수준은 5점 만점에 2.77점이었고, 일반적 특성 중 직업 유무(t=2.25, p=.027), 주관적 경제 상태(F=6.44, p=.002), 주관적 건강 상태(F=23.63, p<.001)에 따라 유의한 차이가 있었다. 극복력은 감사성향과 유의한 양의 상관관계가 있었고(r=.724, p<.001) 스티그마와는 유의한 음의 상관관계가 있었다(r=–.479, p<.001). 위계적 회귀분석 결과, 감사성향(β=0.58, p<.001)과 스티그마(β=-0.19, p=.001)는 극복력에 영향을 미치는 요인으로 나타났고, 이들 변인의 극복력에 대한 설명력은 35.1%이었다. 결론적으로, 혈액투석 환자의 극복력 향상을 위한 전략은 감사성향을 증진시키고 스티그마를 감소시킬 수 있는 프로그램을 개발하여 적용하는 것이 필요하겠다.
