Sequential Clinical Outcomes and Prognostic Factors of Manipulation under Anesthesia for Refractory Frozen Shoulder

Abstract

The aims of this study were to investigate the sequential changes in clinical outcomes with complications following manipulation under anesthesia (MUA) in patients with refractory frozen shoulder (FS) and to identify prognostic factors affecting post-MUA outcomes. A total of 227 shoulders were included, with a mean follow-up duration of 45.1 months. Pain and functional scores showed significant improvements from baseline to final follow-up. The visual analogue scale pain score, the American Shoulder and Elbow Surgeons’ scale (ASES) score, and the subjective shoulder value showed marked improvements. Sequential assessments at 1 week, 1 month, 3 months, 6 months, 12 months, and final follow-up demonstrated progressive improvement. No serious complications or adverse reactions were reported following the procedure. Multivariate analysis identified high body mass index, bilateral involvement, longer symptom duration, diabetes mellitus, and thyroid disease as independent prognostic factors associated with inferior outcomes. MUA is a safe and effective procedure that provides rapid pain relief as well as function recovery for refractory FS.

본 연구의 목적은 불응성 동결견 환자를 대상으로 마취하 도수조작술 시행 후 통증 및 기능 회복의 순차적 변화를 평가하고, 시술 후 임상 결과에 영향을 미치는 예후 인자를 확인하고자 하였다. 총 227예를 분석하였으며, 평균 추적 관찰 기간은 45.1개월이었다. 시각 통증 척도(visual analogue scale), American Shoulder and Elbow Surgeons’ scale 점수, 주관적 어깨 기능 점수(subjective shoulder value)를 이용하여 시술 전과 시술 후 1주, 1개월, 3개월, 6개월, 12개월 및 최종 추적까지 평가하였다. 시술 후 통증과 기능은 지속적으로 향상되었고, 중대한 합병증은 보고되지 않았다. 다변량 분석을 통해 당뇨병, 갑상선 질환, 증상 기간, 체질량지수, 양측 병변이 있는 경우 불량한 예후 인자로 확인되었다. 본 연구는 마취하 도수조작술이 불응성 동결견 환자에서 통증을 빠르게 완화시키고 기능을 회복시키는 안전하고 효과적인 시술임을 보여준다.
