입원환자의 손위생 이행에 영향을 미치는 요인

Abstract

본 연구는 손위생에 대한 입원환자들의 지식, 태도, 이행을 확인하고, 입원환자의 손위생 이행에 영향을 미치는 요인을 파악하기 위해 시도되었다. 본 연구는 상급종합병원에 입원중인 성인환자 165명을 대상으로 2023년 3월 6일부터 3월 17일까지 자기 기입 방식의 설문조사를 통해 자료를 수집하였다. 자료분석은 SPSS/WIN 27.0 프로그램을 이용하여, 빈도와 백분율, 평균, 표준편차, 독립 t-검정, 일원분산분석, Scheffé 사후검정, 피어슨의 상관계수, 다중선형회귀분석을 이용하여 분석하였다. 본 연구대상자의 손위생 관련 지식은 총 12점 만점 중 평균 9.02±1.81점이었다. 손위생 관련 태도는 –3점에서 +3점 중 평균 1.68±0.90점이었고, 입원전 손위생 이행도는 5점 만점 중 평균 3.83±0.67점이었고, 입원생활 중 손위생 이행도는 5점 만점 중 평균 3.36±0.75점이었다. 입원생활 중 손위생 이행은 손위생 지식(r=.415, p<.001), 태도(r=.206, p=.008), 입원전 손위생 이행(r=.647, p<.001)에 대해 양의 상관관계를 나타냈으며, 입원중 손위생 이행에 영향을 미치는 요인은 입원전 손위생 이행여부(β=.588, p<.001), 지식(β=.154, p=.019), 보호자 손위생 시행여부(β=.144, p=.023), 다른환자의 손위생 시행여부(β=.127, p=.048), 순이었고, 영향요인의 설명력은 49.5%이었다. 일상생활에서의 손위생 이행, 손위생 지식, 주변 환경과 타인의 행동이 환자의 손위생 이행에 중요한 역할을 한다는 점을 고려할 때, 의료기관 내에서 입원환자와 내원객 대상 손위생 교육과 손위생에 대한 지원적 환경 조성 등을 통해 입원환자들의 지식 향상과 의료기관 내 환자 손위생 강조 문화를 조성하는 것이 필요하다. 또한, 일반 대중을 위한 손위생 교육의 중요성과 대중 인식 캠페인이 지속적으로 이루어지는 것이 필요하다.

This study assessed inpatients’ knowledge, attitudes, and compliance with hand hygiene practices, aiming to identify factors influencing compliance during hospitalization. A total of 165 adult patients at a tertiary general hospital participated in a self-administered survey conducted between March 6 and March 17, 2023. Data analysis (SPSS/WIN 27.0) included frequency and percentages, mean and standard deviation, independent t-tests, one-way analysis of variance, Scheffé post hoc test, Pearson’s correlation, and multiple linear regression. Participants scored 9.02±1.81 (out of 12) on hand hygiene knowledge, 1.68±0.90 on a -3 to +3 attitude scale, and 3.83±0.67 (out of 5) on hand hygiene compliance before admission. During hospitalization, compliance averaged 3.36±0.75. Positive correlations were found between compliance and knowledge (r=.415, p<.001), attitude (r=.206, p=.008), and pre-admission compliance (r=.647, p<.001). Multiple regression identified pre-admission compliance (β=.588, p<.001), knowledge (β=.154, p=.019), caregiver hand hygiene practices (β=.144, p=.023), and other patients’ compliance (β=.127, p=.048) as significant predictors, explaining 49.5% of the variance. Considering the implementation of hand hygiene daily, knowledge of hand hygiene, the surrounding environment, and behavior of others, as they play an important role in the implementation of hand hygiene by patients, it is necessary to provide support for hand hygiene education, especially for hospitalized patients. It is, therefore, necessary to improve the awareness of hospitalized patients by creating an environment that facilitates handhygiene. Additionally, it is necessary to continue promoting public awareness campaigns regarding the importance of hand hygiene education for the public.
